If so, West Berkshire Council has an exciting opportunity for a Social Care Practitioner to join our Adult Social Care Service.

Reporting to the Locality Manager, you will play a key role in delivering high-quality social care services to our local community.

Your responsibilities will include:

  • Undertaking assessments and reviews of individuals' care and support needs
  • Developing and implementing care plans, ensuring they are person
  • -centred and promote independence
  • Providing advice, guidance, and support to individuals and their families
  • Collaborating with external agencies and professionals to coordinate holistic care

To excel in this role, you should possess the following skills and attributes:

  • A relevant professional qualification in social work or social care
  • Experience in assessing and supporting individuals with a range of care needs
  • Strong communication and interpersonal skills, with the ability to build rapport and establish effective working relationships
  • Sound knowledge of relevant legislation and best practice in adult social care
  • The ability to work autonomously, manage priorities, and make informed decisions

This role requires the incumbent to have a car available at work to undertake his/her duties as they will be expected to undertake at least 1500 business miles per year. Essential Car User status attracts a lump sum allowance paid on a monthly basis as well as a free car parking space if he/she works at one of the Council’s corporate buildings.
