Vacancy Description

Are you passionate about making a positive impact in the lives of vulnerable individuals? Do you have experience in team leadership within a social care setting? If so, we have an exciting opportunity for you to join our dedicated Adult Social Care Service at West Berkshire Council.

As a Support Outreach Worker, you will play a crucial role in assisting the Resource Centre Manager in delivering high-quality care and support to our service users. You will be responsible for:

  • Conducting assessments and reviews to identify the needs of individuals
  • Developing and implementing person-centred support plans
  • Providing practical assistance and emotional support to service users
  • Collaborating with external agencies and professionals to ensure holistic care
  • Monitoring and evaluating the effectiveness of support services

To excel in this role, you should possess the following skills and attributes:

  • Previous experience in a similar care role within a social care environment
  • A compassionate and empathetic approach towards vulnerable individuals
  • Excellent communication and interpersonal skills
  • Strong organisational and time management abilities
  • The ability to work effectively both independently and as part of a team
