Personal Adviser to join the 16+ Leaving Care Team

Do you have the skills, knowledge and experience to work with young people leaving care? Have you experience working with young people seeking further education, training or employment? Are you ready to work in an established and experienced team?

The 16+ Leaving Care Team works to advise, support and befriend young people from the age of 16 who are looked after and those young people who have recently left care (up to the age of 25) with a diverse range of complex needs, to enable them to manage the transition into early adulthood.

We have an opportunity for a Personal Advisor with extensive experience and a passion for supporting young people with complex needs to achieve their potential. You will be required to able to work in partnership with young people to robustly assess their holistic needs and to work with them to develop and review their Pathway Plans.

As a committed and flexible team member you will manage a caseload and work as part of a duty system.

An NVQ Level 4 in Health & Social Care or relevant qualification is required and experience of working with young people in youth work or social care settings is essential

Applicants must be able to demonstrate all essential requirements as listed in the job profile.

This is a fixed term role for 2 years – funding limited project

This role has been identified as public facing in accordance with Part 7 of the Immigration Act 2017; the requirement to fulfil all spoken aspects of the role with confidence in English applies.
